A mole might be covered if the dermatologist believes that it might become cancerous. A blemish will more than likely NOT be covered by health insurance.
Cosmetic procedures are usually not covered by health insurance policies.

I would ask the dematologist to go ahead and contact your insurer for pre-approval… If he has experience with the carrier, he will know what needs to be done…

Usually, insurance will cover “minor cosmetic surgery” for reconstructive purposes – like fixing a fractured nose. Removing a mole is usually done by a dermatologist, or dermatolgy surgeon, instead of a plastic surgeon.
The following are the only conditions for which cosmetic procedures are typically covered:
1) Congenital birth defects
2) Accident/injury
3) Breast reconstruction following cancer surgery
